Our Medical Social Services aim to provide any medical, psychological, emotional, physical, or social assistance to our clients related to their adverse health conditions.
When families do not have the necessary resources and support during difficult and challenging times, our social workers can provide essential services. They will arrange all available resources to promote each client’s healthy lifestyles, preventing diseases, and addressing significant social and emotional factors related to their health problems to achieve their overall well-being. These resources include:
- Patient Counseling and Education
- Psychosocial Counseling
- Discharge Planning
- Grief Counseling
- Patient Advocacy
- Case Management
- Appropriate Referrals
- Offering support groups
Situations will be assessed, and our medical social workers will collaborate with other medical professionals such as doctors, nurses, discharge coordinators, administrative staff, and physical therapists to provide an individualized discharge plan.
